If you are applying for new ration card, follow the below mentioned steps which are provided for your convenience.
If you are applying for !st time , you will apply in the prescribed form D-1 either in the office of District Food and Supplies Controller/Assistant Food and Supplies Officer.
These application forms are available in the office of District Food and Supplies Controller/ AFSO/IFS.
- Two attested photos of passport size
- An affidavit declaring that he has not got prepared any ration card anywhere in India earlier.
- Any Proof of Address / Identity..
List of Documents Required for Ration Card
Attach the above mentioned documents and handover it to the office person / operator in the center and get the acknowledgment slip.
The Sub-Inspector/Inspector will verify the particulars by physical visiting the residence of the applicant and making necessary enquiries. After considering the recommendation, the DFSC/AFSO will issue the ration card.

- The applicant have to pay Rs 10/- as application fees for Ration Cards process.
- BPL families are exempted from application fees for Ration Card.

How to Get Duplicate Ration Card ?
First of all collect application form from the the office of DFSC/AFSO/IFS.
Fill up the application form and attach required documents along with application form and submit it to the office of District Food & Supplies Controller/Assistant Food & Supplies Officer/Inspector Food & Supplies in whose jurisdiction his residence falls.
Necessary Document for Duplicate Ration Card
- Two attested photos passport size of Family
- Report of Depot holder and two photocopies of Rs 5/- as penalty.
The ration card can be collected by the applicant from the office of District Food and Supplies Controller / Assistant Food and Supplies Officer /Inspector Food and Supplies.
Modification in Ration Card
Collect application form from the the office of DFSC/AFSO/IFS and attach required documents along with application form and submit it to the office of District Food & Supplies Controller/Assistant Food & Supplies Officer.

For Member Deletion : Death Certificate / Transfer certificate / Marriage certificate
For member Addition : Birth Certificate
